Misdirected faxes resulting in unauthorized access to personal health information.
Resolved through informal resolution. The health information custodian retrieved the faxes and received confirmation that no copies had been made. The health information custodian also reviewed its practices and discontinued the use of fax machines for this particular purpose.
Section 12(2) notification was fulfilled by way of written letters to the affected individuals.
