Resolved through informal resolution. The health information custodian and the courier company reviewed and revised its procedures relating to the transportation of packages containing laboratory specimens.
The health information custodian worked with the affected individuals’ family physicians to provide verbal notification of the loss of their personal health information and confirmed that all of the affected individuals were notified.
