Loss of personal health information due to theft of a laptop computer from a staff member’s vehicle.
Resolved through informal resolution. The health information custodian reminded its staff that laptops should be locked in the trunk of vehicles, and began to revise its policies regarding document management, including the goal of de-identification of information.
The health information custodian fulfilled the notification requirement by way of written letter to the affected individuals.
