Former Prescribed Persons under PHIPA

The practices and procedures of these former prescribed persons were reviewed and approved by our office. They are no longer approved to operate as prescribed persons under PHIPA and their status has been revoked from the regulations.

London Health Sciences Centre in respect of the Ontario Joint Replacement Registry

 

aSworn affidavits were not required for the 2005 and 2008 reviews and approvals.
bIn 2014, 2017, and 2020 its report and subsequent approval letter were combined to address both its authorities as a prescribed entity and a prescribed person under PHIPA.
cIn 2023, the approval letter to Ontario Health addressed its authorities as a prescribed entity, person, and organization under PHIPA.
dThe initial approval was granted for a period of less than three years so that the timing of the next review would be synchronized with the three-year reviews of the other prescribed persons and entities. eInitially, Cardiac Care Network of Ontario received approval in respect of its registry of cardiac services. The registry was expanded to include vascular services in 2017.
fAs of January 1, 2020, a prescribed entity under the Coroners Act may not collect personal information under its authority as a prescribed entity unless the Chief Coroner has entered into an agreement with the prescribed entity, and the prescribed entity has in place practices and procedures approved by the IPC. Although three entities have been prescribed, only ICES has initiated a review under the Coroners Act and had its practices and procedures approved by the IPC . The other two prescribed entities have not initiated a review under this Act.
gAlthough two entities have been prescribed under the CYFSA, they have not initiated a review under this act. As of January 1, 2021, a prescribed entity under the CYFSA may not collect personal information under its authority as a prescribed entity unless it has in place practices and procedures approved by the IPC.
