Diagnostic reports gone missing
- Hard copies of diagnostic reports were not filed in the patients’ health records and were likely disposed of by one of the health information custodian’s employees.
- Resolved through informal resolution. The personal health information contained in the reports was not lost, as it was also in electronic form. Copies of the missing hard copies were reproduced by the custodian and filed in the appropriate health record.
- The custodian fulfilled its obligations under PHIPA with respect to notification by informing the patient, in person, at their next appointment with their health care provider.
