Resolved through informal resolution. The health information custodian retrieved the individual’s personal health information and ensured that no copies were made. The health information custodian also revised its practices to limit the amount of personal health information contained in health card re-registration notices.
The health information attempted to, but was unsuccessful in notifying the affected individual, as she could not be located.
